Background Study and Health Document Requirements
Requirements
Lake Superior College requires background studies of its students in selected health-related programs prior to final acceptance into the program. Acceptance into one of these programs is conditional upon successful completion of the required background checks. Once you have received notification of acceptance to your health-related program, you will be required to complete the background studies required by that program.
Failure to complete the program required background studies in compliance with the date stated by the program will result in the suspension of your acceptance to the program. Final acceptance to the program and program progression is conditional upon successful completion of requirements for background studies and health documents as required by your program. Please see your specific program information for background study and health document requirements.
It is essential that you submit this information by the date required for the program to which you are applying. Your health program must be able to document the required background studies and health documents required for healthcare clinical site experience.
Criminal Background Study
The criminal background study must be completed by Verified Credentials, Inc. The Verified Credential, Inc. website is available via the link located on this site. Please follow the instructions as directed on the Verified Credentials, Inc. website. You will be required to provide identifying information, as well as a payment source for the $50.00 fee. You will be directed through the application process by Verified Credentials. Results will be returned to you and also to the Division of Allied Health and Nursing at Lake Superior College.
Minnesota Department of Health and Human Services Background Study
The Minnesota Department of Health and Human Services background study form is available via the link located on this site. To apply for this study, please submit your completed form to the Allied Health Administrative Assistant, who will process your application. This background study is free of charge to students.
State of Wisconsin Caregiver Background Study
The State of Wisconsin Caregiver background study form is available via the link located on this site. A $7.50 fee, payable at the LSC Student Payment Office, is required for this study. To apply for this study, please submit your completed form, along with your receipt of payment for your $7.50 fee, to the Allied Health Administrative Assistant, who will process your application.
